Reintegration Case Manager

Job Description - Provides oversight and guidance through case management services to assigned at-risk juvenile clients in a community-based program. The position has direct contact with the client and family multiple times per week in various localities throughout Philadelphia.

The goal is that the client has successful completion of the program and reintegration back into the community.

PRIMARY FUNCTIONS OF THE JOB

1.    Request from the PJJSC social services team or DHS program manager documentation of program completion/certificate of completion while the youth was in detention.

2.    Obtain a copy of a release of information from the youth and parents to obtain copies of any behavioral health reports and school information, while at PJJSC.

3.    Attend discharge planning meetings. Complete and submit discharge planning meeting reports to appropriate partners.

4.    Complete and document all contacts in accordance with reintegration scope and standards.

5.    Meet with the family and youth face-to-face in their home and document a home check/evaluation during the initial home meeting. (the home check must be attached to the pre-discharge meeting note).

6.    Have weekly face-to-face contact with the youth at least twice a month in the home and twice in the community.

7.    Accompany the youth in the community to referral linkages as needed to guide, coach, and support. 

8.    Have weekly contact with the parents/guardians to include face-to-face contact with the family twice monthly, at minimum. Supplemental outreach to parents/guardians via phone is encouraged to share progress and updates in real time. 

9.    Attend school meetings, obtain school attendance, and report cards to monitor youth’s academic progress and advocate for any academic needs.

10. Develop and complete a workforce development assessment and plan, court reports and the ISP. All plans have the involvement of the youth and support of parents/guardians.

11. Refer, coordinate linkages to services for the youth such as but not to, limited to behavioral/mental health, prosocial activities, workforce development/career readiness programs, etc., monitor for progress and completion. 

12. Communicate, if applicable, with JPO, and CUA case manager and participate in meetings, obtain CUA Single Case Plans, the JPO’s Graduated Response Case Plan (GRCP), and provide them with copies of the transitional planning reports and the Individual Service Plan. 

13. Contact the DHS Program Manager to request a Reintegration Oversight Committee meeting in the event the youth require support with compliance.

14. When there is no involvement with CUA or JPO, the Reintegration Worker will submit a copy of the transitional planning report to DHS Program Manager and provide monthly updates.

15. Attend court as required and document outcomes.

16. Maintain weekly communication and working relationships with probation officers and other partners as required based on the scope and standards.

SECONDARY FUNCTIONS OF THE JOB

1. Perform other duties similar to the above as the need may arise.

EDUCATION AND EXPERIENCE:

Bachelor’s degree in criminal justice, criminology, social work, sociology, psychology, counseling or related field and two years of experience with youth or Master’s degree required. At least one year of work experience providing direct social services to youth in a similar setting. Advanced proficiency with word processing and spreadsheet applications is necessary. Working knowledge of virtual communication platforms to meet with clients, families and partners. An acceptable driving record and reliable, and insured transportation required.

Job Type: Full-time

Pay: $20-21 per hour
